Requirements:

1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture
   (CS Form No. 212, Revised 2017 which can be downloaded at www.csc.gov.ph) and
   Work Experience Sheet;
2. Performance rating in the last rating period (if applicable);
3. Eligibility or Board Rating (applicants for positions that do not require the practice of
   profession), or Valid License (applicants for positions that require the practice of
   profession;
4. For Aircraft Mechanic applicants, aside from the CAAP license, submit an
   authenticated copy of Knowledge Test Report;
5. Authenticated copy of Transcript of Records/Diploma;
6. Birth Certificate/Marriage Contract (PSA); and
7. Complete Local Clearances (Barangay Clearance, Mayor’s Clearance, Police
   Clearance, RTC Court Clearance, NBI Clearance, and D.I. PNP Clearance)
