Personal Data Sheet (PDS) – CSC Form 212 Revised 2017 is used when applying for jobs in the government. As a matter of fact, it is the primary requirement that the government agency asks first. It serves as a resume where your important details are written like your personal information, the school where you’ve graduated, your work experience, training, and your eligibility.
It can be downloaded on the CSC website or you can choose to download it here. When printing the form, you should specify the paper size at 8.5 x 14 inches. But since the pandemic started, government agencies prefer the applicants to pass it on their email address to reduce the risk of transmission of the virus. So, you can also fill it up directly on your computer and save it afterward.
It is in utmost important to fill out the form completely and accurately. In fact, gov’t offices are strict when it comes to incomplete PDS forms. It may be the reason for your disqualification.
