PhilHealth offers online services for its members. You will be able to see your contributions online, get an MDR or Members Data Record, etc. No need to go to the PhilHealth office just to get it. All you need is an internet connection.

What is an MDR?
MDR is an acronym for Member Data Record. It is needed especially if you need to claim your hospitalization benefits. This will serve as proof that you are indeed a member and are entitled to the benefits.
This document usually consists of member’s information, name of employer (if employed), beneficiaries and your payment information.
How to Get an MDR Online?
1. Register for an Account
Firstly, you need to have an Online Account. We make some guide for Philhealth registration online here.
2. Log in to your account
After completing the registration and activating your account, you can now log in to the portal by using your Philhealth Identification number and the password you make. Enter the captcha and click Log-in.
3. Click Print MDR
After you log-in you will then see your information on the screen. Just above your details, there you can click the “Print MDR”. Click that button and it will show you the copy of your MDR. In the upper right corner, you can see a printer icon. Click that icon to print your Members Data Record.
