• IT Graduate or other Related Course
• Radiological Technologist
• Laboratory Technician
• Medical Technologist
• Nutritionist-Dietician
• Nursing Attendant
• Psychometrician
• Nurse

Initial Requirements:

• 3×5 colored picture (Whole Body)
• Personal History Statement (Original 2×2 picture)
• PSA copy of Birth Certificate (Original with OR)
• Copy of Form 138 / Transcript of Records
• Copy of High School/College Diploma/TESDA Certificate
• AFPSAT
• Medical/Physical Examination Result
• Clearances (Barangay, Police, Mayor, NBI, RTC)
• Other Certificates, if any
• Height requirement (Minimum of 5’8 for Philippine Army Applicants)
