If you are applying as a Patrolman/woman in the PNP, you will need an Affidavit of Undertaking and Affidavit of Confirmation as a part of the requirements.

Instructions

These instructions are for filing of Affidavit of Confirmation.

  1. The applicant should not copy and paste its contents as it is a standard format.
  2. No one is allowed to alter, change, remove or delete any portion of these affidavits. The altered affidavits will not affect any violation of the confirmation under the original affidavits shown in this document.
  3. The applicant must personally fill the form by “PDFfiller” (an application available in both iOS and android), by “Adobe Acrobat Reader DC” using its “fill and sign” function, or, directly, by writing by hand above the line provided for the required data.
  4. Both “PDFfiller” and “Adobe Acrobat Reader DC” are free of charge and this office enjoins or forbids you to make any in-app purchases.
  5. The data to be filled in are the following:
    a. Under “1”, the city where you subscribed and sworn the affidavits
    b. Under “2”, your full name, which must be in the format “First Name Middle Name Surname”. (e.g. Juan Santos Cruz / Maria Sanchez dela Cruz)
    c. Under “3”, your address, which must include the following details:
    (No./Lot/Block/Unit, Street Name, Barangay, City/Municipality, Province,
    Postal Code).
    d. Under “4”, the position being applied for, which include: (i)Patrolman/woman; or (ii) Lateral Entry.
    e. Under “5”, the region you chose as a cluster, which is: NCR, Region 1, Region 7, Region 9, and Region 11. This is where you will be processed and NOT where you are applying. Chose the nearest to your present address.
    i. (e.g. “A” resides in Bohol but he wanted to apply to NCRPO, he must choose Region 7 as cluster but he is still an applicant of NCRPO)
    f. Under “6”, the name of your beneficiaries, your relationship with them, and their date of birth. Your beneficiaries depend if you are Single or Married.

i. For Single. Parents (living), Siblings (Brothers and Sisters, Full or Half-
blood)

ii. For Married. Spouse (Husband/Wife) and Children (Legitimate or Illegitimate).

g. Under “7”, signature above your Full Name. It must be done in own handwriting and NOT typewritten. The electronic signature will not be honored.
h. Under “8”, leave it blank and let the notary public fill this area upon notarization. (This is subject to the Notarial Rules and Practices, which will be enforced by your respective notary public).

  1. After filling the PDF through “PDFfiller” / “Adobe Acrobat Reader DC” or through your own handwriting, print the same on a Long Bond Paper (8.5’x13’).
  2. After printing, the applicant must cause the notarization and sign the affidavits in his or her own hand. An authorized person is not allowed to sign on behalf of the applicant.
  3. Make sure to have the seal of the notary be marked at the appropriate place on your affidavit before uploading it to PNP Online Recruitment.
  4. Submit the original copy of the Affidavits (with notarial seal) together with other
    documentary requirements.


WARNING: This is a sworn affidavit. Any falsehood committed is a violation of Revised
Penal Code, which will cause PERMANENT derogatory record at PNPRSS database.
