How to Apply

Interested and qualified applicants should signify their interest in writing through an application letter addressed to the head of office. Applicants must attach the following documents to the application letter and send these to the address below not later than November 02, 2026.

  1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2025) which can be downloaded at www.csc.gov.ph;
  2. Resume/ CV with Passport size ID picture
  3. Autobiography
  4. Duly authenticated Certificate of Civil Service Eligibility (if applicable)/ PRC License (if applicable)
  5. Duly authenticated RA 1080 (PRC Board Rating- if applicable)
  6. Valid NBI Clearance
