  3. Hard copy or electronic copy of proof of eligibility/rating/license
  4. Hard copy or electronic copy of Transcript of Records.
  5. Hard Copy or electronic copy of Certificate of Employment (Previous Employeer)/Service Records (if there's any); and
  6. Hard Copy or electronic copy of Certificates of Training Attended ( applicable to the position that needs relevant trainings)
