  2. Notarized Personal Data Sheet (PDS) and Work Experience Sheet both forms are donwloadable at CSC Website with applicant's recent photo. PDS should be computerized and completely filled in the required information.
  3. Authenticated Transcript of Records (TOR) by applicant's University/College/School (If applicable)
  4. Authenticated Diploma by applicant's University/College/School (if applicable)

Sealed Neuro-psychiatric Exam Result from MMMH&MC-APU or any accredited Government Hospital Psychiatric Testing Centers – Neuro-psychiatric exam results issued by external testing centers shall be subject to the validation and review by the MMMH & MC Acute Psychiatric Unit (APU). In cases where the submitted results are found to be incomplete, insufficient, inconclusive, or requiring further evaluation, the applicant may be required to undergo repeat Neuro-psychiatric examination at the MMMH & MC-APU.
