  5. Certificate of units earned/TOR/Diploma for those with Master's Degree (if applicable);
  6. Certificate of Employment or Service Record;
  7. Performance Evaluation in the last rating period (if applicable);
  8. Certificate of trainings/seminars attended within the last 5 years, arranged according to date from recent to past;
  9. Published research relevant to the profession, if any;
  10. Achievements (awards/honors,exemplary citations,research,and other trade skills); and
  11. Other professional achievements, if any;
  12. Updated PRC ID/License;
  13. Updated PHIC Accreditation;
  14. Diplomate certificate; and
