4. Photocopy or Electronic copy of Transcript of Records (for Bachelor's Degree and Postgraduate Degree, if applicable);
5. Photocopy or Electronic copy of Diploma (for Bachelor's Degree and Postgraduate Degree, if applicable);
6. Photocopy or Electronic copy of Certificate/s of relevant training and seminars attended;
7. Photocopy or Electronic copy of Civil Service Eligibility or Board Rating. For vacancies involving practice of profession (RA 1080, RA 10029), Valid PRC ID/proof of payment or schedule of appointment for the PRC renewal license; and
8. Individual Performance Contract Rating (IPCR) or Certificate of Performance Assessment (CPA) in the last rating period (if applicable).
