- Performance rating in the last rating period (if applicable);
- Photocopy of certificate of eligibility/rating/license; and
- Photocopy of Transcript of Records.
- Photocopy of Diploma;
- Photocopy of Certificate of Training/s (if applicable);
- Photocopy of Certificate of Employment (if applicable); and
- Service Record (if applicable)
