If you are planning to apply for a UMID ID (SSS ID), you need to fill out the UMID ID Application Form. However, members who applied for an SS Number online beginning December 10, 2020, do not need to fill out the UMID Application Form.

They just need to schedule an appointment for biometric data capture through the My SSS Appointment System. On the day of their appointment, they need to present their printed SS Number application and the transaction number slip.

Those who prefer manual application in branches will still be accommodated, following the branch number coding system.

Write your height in centimeters and weight in kilograms. To convert measurements, remember that 1 foot (ft) equals 30.48 centimeters (cm), 1 inch (in) equals 2.54 cm, and 1 pound (lb) equals 0.4536 kilograms (kg).

5: Distinguishing features
Limit the distinguishing features listed to those that can be found on your face, such as a mole under the right eye or a birthmark on the left cheek or forehead. Provide accurate descriptions of these features.

6: Mandatory information
Always include the following mandatory information:

  • Country of place of birth: If you were born outside the Philippines, state the country of your birth.
  • Mobile number: If you are applying locally, provide your mobile number.
  • Email address: If you are applying from abroad, include your email address.
  • In case you cannot provide a required mobile number or email address, provide the mobile number or email address of an immediate family member that SSS can use to communicate with you.

7: Card Replacement and fee payment
For any type of card replacement, you will need to pay the required fee at an SSS branch office, accredited bank, or collecting agent. Write the Special Bank Receipt (SERVReceipt Number/Transaction Reference Number) in the provided field. Submit the completed form, along with the required documents and proof of payment, to the nearest SSS branch office.

8: Unclaimed UMID cards
If you need a card replacement due to unclaimed UMID cards that have been unclaimed for more than five (5) years, you will be required to pay a replacement fee and undergo biometric data recapture.

9: Submitting the form
Once you have completed the form and gathered the necessary documents, submit the form to the nearest SSS branch office. Refer to the “Documentary Requirements Guide” table for a list of the required documents that should accompany your submission.
