PMMA Neuro-Psychological Exam Schedule 2020 Update

PMMA releases the schedule for the Neuro-Psychological exam for those who successfully pass the 2019 PMMA Entrance Examination.

Update: March 11, 2020

PMMA on its official Facebook account announces the postponement of Neuro-Psychological Evaluation starting March 12, 2020, until further notice, to fight the spread of Corona Virus in the Philippines.

Update: June 5, 2020

The PMMA announces the resumption of the Neuro psychological exam 2020 on its official Facebook Page.

An online neuro examination will be adapted instead of the usual pen and paper test to comply with the health protocols regarding the COVID 19 guidelines of the government.

The online neuro-psychological examination is scheduled for June 8-12 only. Call the appropriate SERVICE PROVIDER during office hours (Monday to Friday, 8 am-12 noon, 1 pm to 5 pm) for instruction, clarification, and schedule date of exam.

Guidelines & Important Reminders for Neuro Psychological Exam 2020

1. The results of the PMMA Neuro-Psychological Examination shall be announced on the same day of the applicant’s preferred Neuro-Psychological examination schedule. Hence, those who will successfully pass the Neuro-Psychological Examination are hereby advised to report the following day for their PRE-MEDICAL and LABORATORY EXAMINATION.

2. Those who will successfully pass the Neuro-Psychological Examination are required to submit a photocopy of their respective grades on the day
 Grade 12 Examinee- must present his/ her Grade 11 report card
 Senior High School Graduate- must present his/ her Grade 12 report card
 College Level- must present his/ her latest transcript of records (TOR)
 College Graduate- must present his/ her official transcript of records (TOR)

B. What to do before the examination

1. Online registration will open on February 14, 2020. Applicants need to register online to secure a schedule. Onsite verification of attendance will open at 6:00 AM.
2. Visit the PSYCH SYSTEM AND INNOVATION, INC website and INTELLIMETER INTERNATIONAL RESOURCES INC for online registration stated above.
3. Registration Fee for Neuro Exam – Php1, 100.00

C. What to bring on examination day

1. Original and photocopy of Confirmation form with the Payment Slip and one (1) valid I.D. (school ID or any government-issued I.D) should be presented on the day of the test. (No registration form, no examination); and
2. Two (2) pencils and erasers (Not sign pen)

D. Important Reminders

1. Applicants must wear a PLAIN WHITE T-SHIRT with or w/out collar (Sleeveless Shirt, Short pants, ripped jeans and slippers are not allowed).
2. Strictly, there will be NO changing of schedule.
3. Strictly, follow the assigned Neuro Psychological Service Provider.
4. Applicants are REQUIRED to attend a briefing regarding cadets’ life in PMMA to be conducted by the Chairman, Admissions Office after the test.
5. No latecomers are allowed to take the test. All applicant examinees should be in the testing room at 7:00 am
6. Psychological Evaluation shall be taken in one (1) day only.
7. NO confirmation, NO examination.
8. Eat a heavy breakfast. Drinking and eating inside the testing room are strictly prohibited.
9. Mobile phones and electrical gadgets should be turned off while inside the testing room
10. Parents are STRICTLY PROHIBITED inside the testing room.
11. Applicants are only allowed to take the examination ONCE.
12. Violation of these policies will be a ground for the applicant’s disqualification.
13. Applicants have to pass Neuro-Psychological Evaluation, get the APPLICANT’S ROUTING SLIP to proceed for the PRE-MEDICAL (HEIGHT, WEIGHT, ISIHARA, SNELLENS, DENTAL EXAMINATION) before he/she can proceed with the LABORATORY EXAMINATIONS.

PRE-MEDICAL/PRE-DENTAL EXAMINATION and LABORATORY EXAM

A. SCHEDULE AND ADDRESS

1. What to bring on the PRE-MEDICAL Screening (Height, Weight, Ishihara, Snellens) and Laboratory Examination Day
 Any Valid Identification – Driver’s License/Recent School I.D./Passport/NBI Clearance. (Submit a photocopy of the applicant’s I.D. to the service providers. NO I.D., NO EXAM);
 Black ballpen;
 Applicant’s Routing Slip and Official Receipt for PRE-MEDICAL (to be given and paid at the Neuro-Psychological Examination Venue) before she/he can proceed to the ACCREDITED CLINIC.; and
 Two (2) pcs. 2×2 COLORED PICTURES WITH NAME TAG (White background).

2. Reminders
 Applicants without routing slip and receipt shall not be attended to at the clinic for PRE-MEDICAL.
 APPLICANTS HAVE TO PASS THE PRE-MEDICAL Screening before he/she can proceed for the PRE-DENTAL EXAMINATION.
 If an applicant is declared qualified, he/she can then continue and proceed to the LABORATORY EXAMINATION.
 Applicants must wear a PLAIN WHITE T-SHIRT with or w/out collar (Sleeveless Shirt, Short pants, ripped jeans and slippers are not allowed).
 Applicants must be at the clinic at 6:00am-7:00am or earlier.
 Pre-Medical and Laboratory examinations will take about 1-2 days.
 Parents are STRICTLY PROHIBITED inside the Clinic.
 Applicants are only allowed to take the examinations ONCE.
 Violation of these policies shall be a ground for the concerned applicant’s disqualification.

3. Enrollment as PROBATIONARY MIDSHIPMAN
Once an applicant has qualified/passed the medical screening, he/she should report to the Admissions Office and submit the following documents at the PMMA Registrar’s Office starting June 01, 2020 until June 30, 2020:

 Senior High School Students Permanent Record DepEd 137-SHS (submit if issued; if not School to School Request) Note: NO NEED TO SUBMIT IF WITH COLLEGE BACKGROUND

 Senior High School DepEd Form 138 Note: NO NEED TO SUBMIT IF WITH COLLEGE BACKGROUND

 Honorable Dismissal or Certificate of Transfer Credentials (with College Background)

 Birth Certificate: Original Copy secured from -Philippine Statistics Authority( PSA) and photocopy (colored)

 NBI Clearance

 Certificate of NO Marriage (Cenomar), Original Copy

 Certificate of Good Moral Character from Barangay

 Five (5) Copies of 2×2 Pictures ( with nameplate, white background)

 Signed Parental Consent (Form can be secured from the PMMA Admissions Office and distributed during Laboratory Examination)
