Some countries are requiring proof of vaccination for people who wants to enter their territory. Thus Overseas Filipino Workers (OFW) who were vaccinated in the Philippines need to show a vaccination certificate. Local LGUS’s are giving their own card, but it is better to get a BOQ Yellow card or the International Certificate of Vaccination (ICV) for it to be accepted outside the country.

Disclaimer: Not all countries accept the ICV nor it is required when you travel outside the Philippines. It will depend on the country of your destination. Better to check the requirements of the said country beforehand.

Bureau of Quarantine Yellow Card

The Bureau of Quarantine is issuing a vaccine certificate that is recognized by the World Health Organization (WHO). It is called a yellow card or yellow book because of its color. If you are traveling outside the country that requires an ICV, read the steps on how to get an appointment below.

How to get an appointment to get the BOQ Yellow Card?

The Yellow Card is issued during weekdays in NCR (BOQ Main Office, BOQ Mall of Asia Satellite Office), Luzon (Batangas, La Union, Subic, Laoag, Tabaco), Visayas (Cebu and Iloilo), and Mindanao (Davao and Cagayan De Oro). But before going to these sites, you can book an appointment online. You need to have a copy of your valid ID and your certificate of vaccination (front and back).

Step 1- Visit BOQ Online Booking System

Visit the Bureau Of Quarantine Online Booking System by clicking this link https://icv.boq.ph/.

Step 2- Choose Your Location

Select your Location. The location you choose will be where you will be getting your Certificate. Afterward, click the”Book an Appointment” button and read the terms and conditions. Click Agree to the terms and Conditions.

Step 3- Enter email address

Enter your email address. Make sure that it is active and working. Tap the “I am not a robot” box and click submit.

Step 4- Select BOQ Service Type

Select “Certification” on the Service Category. Prices of Certification Services will be shown. For the Covid Vaccination Certificate, the amount you need to pay is P300. Select the International Certification of Vaccine for Covid 19 then select next.

Step 5- Set your appointment schedule

Set your appointment schedule. The dates are color-coded wherein green means that date is open, red means that date is not available, white means no office. Select from the green-colored schedule. Upon clicking the date, available time and the number of slots will be shown. Check the box for your preferred time.

Step 6- Input your details

Fill out the required information and afterward click “Next”. Submit the requirements by uploading them which include a copy of your valid ID and your vaccination certificate from your local LGU. You also need to input the details of your vaccination certificate as well as your ID number.

Step 7- Payment

Pay the fee using your preferred payment method. You can choose from the following:

  • Debit/Credit Card
  • Cash (7 Eleven, LBC, Family Mart, and Cebuana Lhuiller)
  • E-Money (I.e. GCash, PayMaya, and Cliqq)
  • Online Banking

There may be an additional fee aside from the P300 that you need to pay depending on the method of payment you choose.

Step 8- Success

After payment, you will receive a reference number. Save the reference number or you can print your receipt. You are now finished with getting an appointment. To check your appointment status, click the “View my appointment status” and input your reference number and date of birth.

Things to Remember

You need to bring your vaccination card from the LGU on the date of your appointment. You cannot change the date of your schedule after setting the appointment online. The payment is non-refundable as well.
